Words cannot express how truly grateful I am because of Mr. Scott Kalkin. Unfortunately I had multiple upper extremity surgeries in 2012 which left me disabled and unable to work. I had to utilize my long term disability policy from my employer that I paid for since 2000. I did not understand at the... time that I purchased an Erisa plan which is a very complex and difficult to understand policy.
