As long as the LLC is created all you need is a quit claim from tne individual into the llc

If the note is lost the plaintiff has to advise of that in their complaint but can still sue you. Hope you have hired counsel to represent you

The only way to remove her from the mortgage is to refinance and satisfy that mortgage in full
