The question is not which form but which court. Small claims appeals are heard in the Superior Court, not the Court of Appeal.
I am appealing the judgment. The defendant prevailed with the cost at the hearing. How do I let the court know of the appeal?

If the case actually is on appeal because you properly and timely filed a notice of appeal, then the court knows about it.
Can you subpoena a justice of the Court of Appeal about a case they ruled on if that justice has retired?
