Ask your lawyer. You have a right to know. It may be a company holding a doctor’s lien.
Could my dad open a joint acct w/my sister (500k) that upon his death just becomes hers? outside of the trust? w/no taxes owed?

Yes they account could go to your sister without probate. Unless the entire estate is of a taxable amount, there would be no tax.
