You can change your name through local laws in Florida. You will need to ask an attorney there what his or her fees are and what the filing fee...

Yes - you can change status. Note the H-1B visas are exhausted for the year. So you cannot change to H-1B but you could have otherwise.

You need to have a record of your vaccinations given to the USCIS doctor. He or she will let you know if that is sufficient.
