If you truly are an independent contractor you should receive a 1099 and you can request it from the company.
Does an employee need to sign a waiver for the 2nd meal period every time they are waiving it during that week or month?

Also, keep in mind that if the employee waives the first meal period then the second meal period cannot be waived.
