Any previous immigration violations must be disclosed at the interview. Working without permission, alone, should not give rise to inadmissibility...

Not necessarily. You can still maintain your status with Company A as long as you do not violatethe terms of the petition.

As an "immediate relative" your wife would not be disqualified from adjusting status solely because her I-94 has expired.

You do not need to file a new I-485, but you must advise the USCIS that you have taken a change in direction.
