You must tell the truth to questions posed by immigration officers and on immigration forms. Failure to do so will inevitably lead to problems....

Your friend needs to retain an experienced immigration attorney. Nothing in your posting indicates that adjustment of status is possible.

How do i properly have an affidavit notarized? Does the writer only sign and date it in the presence of the notary?

Notarization is notarization of the signature, not the content.
