You can petition to the court for a name change. You have to prove your honestly changing your name instead of trying to avoid creditors, etc.

Communicate with him. You can only file contempt for willful violation of a court Order. If it's not ordered, then no contempt.

Not technically, but you must follow any relocation language stated in an Order.
