Supervision is not a conviction. However, if the application asks, have you ever pled guilty to a misdemeanor or felony you must answer yes.

My license was permanently revoked in the State of Kansas in 2004 for a 4th DUI. I moved to The State of Texas in 2010.

Not in Illinois but try Alabama or colorado.
If I got a DUI in Illinois, while being licensed in MN, will it affect my MN DL?? Will it show up on my driving record?

Illinois will report the dispostion to MN, even if you receive supervision. What MN will do with that information I do not know. Let your MN lawyer...

Check with the PA DMV. They are unlikely to issue a license. At best, they may issue a temporary one to give you a chance to clear up the Ill DUI....
