You employer needs to file for the extension. Since you may be gone for a year you may consider a new H1b rather than an extension.

The first interview date is not set by Cleveland. If you are set for a date you cannot appear you may ask Cleveland to re set.

You can apply for a visitor visa but is very unlikely to be granted. The US consulate is likely to believe your husband is coming to United States...

If you have a home resident requirement you will need a waiver or wait two years. if no home resident requirement you may return as early as you like.

You must meet in person within the two years proceeding filing for a fiance visa or you must marry. There are exceptions but they are very limited.

The date on my ds-2019 is different from on i-7979A, which on of them is the date that j1 visa issued on.
