Emplyer must notify the government within a week or 10 days.
The USPS lost my DACA work authorization approval card. USCIS told me I would have to reapply and pay the $495 all over.

Send a letter to the USCIS office who supposedly sent your card and inform them that you never received the card. Ask then to issue you another.

If you file for change of status, you may continue OPT employment until July 15, 2017. Do speak to the school designated official.
