A w-2 should only be generated if you worked for the company. Contact the company if there was an error and then contact the IRS.

As long as the period of unlawful presence does not reach 180 days there is no bar to re-entry.

Under active review means it is being reviewed and you can expect an answer when USCIS finishes their active review. But do not hold your breath.
