Try Ombudsman's office first. The case might be pending because there were some staff changes at USCIS office at 26 Federal plaza.

It depends on many factors, including your criminal record, if you used Visa Waiver to come to the US, if you had prior Immigration problems, etc.

If the first I 485 is no longer pending, your EAD is not valid. It stopped being valid when the I 485 was denied. You need to refile everything.
