You can find a form Application for Probate of Will as a Muniment of Title in the Bexar County Law Library. Look for the "Texas Transaction Guide" by Dorsaneo. Before you do that, though, why do you want to probate the will as a muniment of title? Has it been more than 4 years since the date of death?
