My wife is American citizen by birth but she applying for her first passport ?
My wife is an American citizen ,and she is applying for her first USA passport , but on her birth certificate only her mother name is mention and she never known her father nor her mother tell her about her father when she grew up , my question here is that on her Passport Form DS-11 , there is a question to mention your father name should we leave that empty on the DS-11 form , and do the US post office agent will ask about it that why you left this part empty on the form , also the US post office agent will keep my wife orginal birth certificate and send it with DS-11 form to USA State department or they only send photocopy my wife birth certificate ?
Generally a US passport applicant could write 'unknown' for an unknown parent. The U.S. Postal Service will require her original birth certificate. They are to mail back the original birth certificate back to her address.
