You have to get a court order for visitation then enforce it with the court.

No that is not proper procedure. A court reporter is not to be used in that way.

The court will likely not accept the 5 year old form. You will have to file for termination of parental rights then adoption.

Supervised visitation at a center is not the best idea, especially for older children. Plus they have limited space available.

Assuming the witnesses have personal knowledge of the facts of the case, then yes subpoena them to court, always a good move.

Switching judges will not help you. Use FL Dept of Revenue or a lawyer to enforce the award.
