Unless you have an advance parole travel document, which will be difficult to obtain, you can travel in the US only.

Wait until you become a citizen, then petition for your mom. OR Get here here and have her apply for asylum.

Yes, she can give birth here. Here visa would allow her to potentially come in. Yes you will be responsible for the medical costs.
