DO I have to leave US if decision on I360 is still pending and I94 is about to expire? Or can I still apply for an extension?

You have to either extend R-1 or leave US because pending I 360 doesnt authorize you to stay in US unless you have legal stay.

ACD shouldnt be a problem if there is no prior records but I would suggest you to hire an immigration lawyer who can represent you at the interivew.

1) You have to apply for H-4 with new empoyer
2)You can submit H-1B
3) It will not impact your husband trasfer.
