The FL-141 is, in effect, a Proof of Service. There is no need to serve a Proof of Service on the other side (although most do, anyway).

I don't see any reason why you can't sue the employer. They are supposed to provide the pay records signed under penalty of perjury.

I am not aware of any written requirement that you share this form (Declaration re: Service of Declaration of Disclosures) with the other party....
