birth certificate is not submitted with H4 EAD. It is submitted with AOS. You would provide evidence of the name change. No problem. Even countries...

the H1B will not be rejected without all the vendor information. USCIS may issue a request for information once accepted.

the entry stamp and green card should state the same date otherwise the green card rules.
