While a person on H1B may maintain status pending adjustment to green card an EAD and AP are recommended.

An H4 leaving the US will need to apply for and obtain an F visa to be admitted in F status on return to the US.

There is no immigration related surcharge. Unaware of any US tax related surcharge. US IRS taxes on income not assets or arrival.

The CIS Ombudsman recently stated all EB cases will be interviewed beginning 2019. In the interim EB interviews will be scheduled for those selected.

H1B requires a Bachelor's degree in a specific specialty. An employer might sponsor you for a green card that required an associates.
