The USCIS site can be wrong. An H1B RFE should be answered. The chance of approval are good depending upon the RFE.

An H4 leaving the US will need to apply for and obtain an F visa to be admitted in F status on return to the US.

There is no immigration related surcharge. Unaware of any US tax related surcharge. US IRS taxes on income not assets or arrival.
