See my previous answer. There are two different affidavit of support forms. Check the USCIS website and follow the instructions.

No. You will need to wait until April to file your H-1 visa. However, speak to an attorney as other options may exist.

You must be a permanent resident or citizen. However, your Us citizen or legal permanent resident sponsor can include your income.
