You need to call them to resolve. Not really a issue that can solved with Bankruptcy.

That's should have already been protected with the correct excemptiom. I usually like to have my clients wait till after discharge.

Does submitting a fee waiver affect the automatic stay at time of filing? Will it be in affect after the fee waiver is approved?

The stay will go into effect as soon as you file. The fee waiver will not affect it.
I received a DUI in 2001 I went to DMV to get a 10 year printout. It is still on there why and how can I get it removed

Sorry you cannot get that removed. After 10 yrs it cannot be used as a prior. But will still remain.

Just because you don't have a driver license doesn't mean you can't be charged with a DUI. You need to make sure you talk to an attorney.

You can still apply to the court and the DMV for a critical need restricted license which would allow you to drive to school and to work only.
