I would not expect an interview if the only RFE issue was for a new medical exam.
Should I stop working a day before the EAD expiration date, or can I work on that date?

You may work on the day the EAD expires.

Unfortunately this is not an option. You would need to change status to a work authorized visa and there may not be any that you qualify for.

You may maintain your H-1B until the L-1B is actually approved if you are concerened.
