I am going to my country to have my H1b visa stamped. I have DUI back in 2007, do I need to have medical exam for the visa?

Not likely, but will depend on the individual consulate. Usually medical exams are ordered for multiple DUI convictions.

The only way is to have an attorney file for a Passenger Record Redress request on your behalf. (DHS TRIP)

I-140 is only valid, IF it got approved and not otherwise withdrawn by petitioning employer.
