Dictated by terms of order or judgment. It certainly wouldn't hurt to use last known address.

As my colleagues indicated, unless it is in the judgment, you really are at a loss to enforce an oral agreement.

Sounds like you need a restraining order immediately which would modify the current visitation schedule.
