The husband will be "included" in the petition automatically, since the F-4 category allows for derivative visas to dependents.

Do I need to complete a I 601 waiver if I have remained outside the US for over 10 years? I would like a nonimmigrant visa.

The I-601 only applies if you are seeking an immigrant visa. The I-212 applies if you have been previously deported. Consult with an attorney.
