I agree with the other answers and would just add to your question #3 that you also need a medical certificate and the petitioner needs to submit...

About 6 months from time of filing if there are no requests for more evidence. The time can vary depending on USCIS workload.

Yes, the employer can withdraw its petition at any time. If by "Fresh H1B case," you mean one for the FY2014 quota, then another employer cannot...
