How soon after Chapter 13 dismiss you either, re-file or file for chapter 7; and will the stay order be different?

Normally the stay does not dissolve for 14 days, While you can file when the order is signed, sometimes best to wait couple of days then refile. ...

No, not correct. Travel expenses may be considered in any partition or child support award, but this is not automatic in some statute somewhere.

You would need to get a release in Texas, since you are in another state, that state collections laws govern.

A mailed bill arrived today (10/15/14) from a Dallas hospital covering dates 12/29/09 - 01/18/10. Can they still collect on it?

If less than 4 years they can still sue to get judgment. They can always send a bill, but should not threaten to sue if over 4 years.
