They are looking for evidence to establish your presence so having a bank account would serve that purpose. You can blackout your account number.

What you have is sufficient but if you wish to obtain a copy of that I-94 you can file an I-102 and have a copy sent to you.

I agree with the previous responses. Be sure you file your I-751 ninety days before the expiration date on your conditional green card.

You can apply for them while they are here and since they entered legally you can chose to adjust there status and request a travel permit while...

You need to report them lost. I would recommend you notify NVC that the original passport was lost.
