Simply state the dates you can recall to the best of your recollection and explain that it is a guesstimate as you lost your passport.

There is no problem with him having various bank accounts in the United States if he has lawful status

The provisional waiver law goes into effect on March 4, 2013. It allows the beneficiary to have the waiver approved before going ot hte consulate...
