It means that the address was either incomplete or a duplicate address. Reaffirmation is a completely separate filing.

I've never heard a prosecutor use that exact terminology, but my guess is that it means the offer is 3 years in DOC or 4 years in DOC suspended.

Fractured ankle on 9 year old. Went to ER had to be hooked on IV w pain meds to reset her foot. Wore a hard cast for 5 weeks.
