For form i-751, section 3, question 1, my husband goes by his middle name, should we note that in this section?

It likely will make very, very little difference, but if he uses his middle name as a first name it should be noted on the form.

You can use your husband's last name if you want, it is not a problem. You should see an attorney to discuss your eligibility for adjustment.

You need to have a visa avaialble and be in some lawful, nonimmigrant status to file an I-485.
