After briefing, the Court ... remanded the life insurance claim, finding the insurer's decision was arbitrary and capricious for failing to comply with the plan terms regarding eligibility. The Court also assessed a $74,140 penalty on the plan administrator for the 674 combined days it refused to provide us with the plan documents.
