The original will should be valid since no changes were made. As executor, you will follow the terms of the original will.

It has nothing to do with not being related by blood. It is the way your mother thought best when she asked the attorney to prepare her trust.

Medical liens can be placed on homes in trust. I doubt that the state would sell the home, but the lien would have to be paid when the house was sold.
