I received a summons for a promissory note from 10 yrs ago. The statue of limitations in NC is 5 years. Do I have to appear?

You absolutely want to respond/appear. If you have a valid defense and you do not affirmatively raise it, you will have waived it.

Should have contested the case when the creditor first sued you. If there's a judgment entered and its past the appeal period, you're kind of stuck.

You should file a dispute with the credit reporting agencies if the creditor will not voluntarily remove it as your first step.
