There is no form to fill out. Hire an experienced criminal lawyer and have him contact the prosecutor. Maybe he can persuade them to reduce it...

In a criminal case the public defender culd be appointed. Contact the pd's office and see if you qualify.

absolutely not. To qualify as an assault the threat must be imminent. You cannot have an imminent threat oh the phone.
