It is not required but the more evidence of the continued bona fide relationship the better, so if affidavits are available to you then send them.

Yes, Unlike the L1 who must work on the business, L2 does not have that restriction and may work anywhere.

A copy will work for most things, you can try requesting it from USCIS but you may not get it.
