The release should have covered the owner and driver as well as the carrier. This is standard settlement practice.

I suggest using the date you last worked. As noted , it can be amended later. Some judges like to negotiate the onset date.

If you have workers comp coverage you must report it to them asap. If not, you should retain counsel asap. These causation issues are complicated.
