Huh? It is not illegal to have a 2nd loan, it would be improper to lien the car without the title showing the original and only loan being amended....

What you describe is not malpractice. It may be bad practice and it may be borderline unethical and the Bar may address it if you file a complaint....

You can file a letter asking for the restoration of your rights to the Guardianship Judge OR hire an attorney to pursue the matter for you.
