Very rare. I will have to look at the actual stamp. You can extend your period of stay (for different reasons).

The yellow paper letter is problably a request for evidence or a reminder (in naturalization cases) to bring your identification (license) with you...

Nothing. Just show them your green cards. Once you become a green card holder you are no longer a holder of a nonimmigrant visa.
