If the Judge issued a D6 suspension for the conditions then you would have to complete them in order to get your drivers license back.

Mark is correct, you would need to contact the jail. If you are not getting anywhere with them contact a supervisor.

There is not. There is a difference between driving while license suspended with or without knowledge. With knowledge is a criminal offense...

Yes you will likely violate for any arrest while on probation. The strenght of the case may determine whether or not you can get a bond.

Not in Florida. Your license is forever revoked in Florida after 4 DUI convictions.

I would not. I would hire a lawyer to try and get the bench warrant removed before you travel to that state.
