I don't see a question. They had 120 days to decide. Hire an attorney to file an ina 336 review in federal cour based on the delay.

Yes. Ask your employer's attorney. Could need to seek immigrant visa if extension is denied but there are exceptions.

Probably hire an attorney. You can write and file something, prob I-102. I don't see immigration do many things right and think you may need file...
