Generally, you can get one from the Department of Health in the state in which the deceased passed away.

They do not. I would prepare and file a Request for a Collection or Due Process Hearing (IRS Form 12153).

Do a paper Form 2848 and fax to the CAF Unit at IRS (855.214.7519). At that time, they should assign you a CAF #.
