1099 or W-2? How should a bonus be documented when given 2 years after end of employment & employee with different employer?

I would recommend consulting your accountant. My gut feeling says it should be a w-2 because when the money was earned she was an employee.

More information will be needed. On most savings accounts if you earn interest then this is income and a 1099-INT is typcially issued.
