You can file the I-485 with USCIS - NVC doesn't do the I-485, they do immigrant visa processing.

When were you terminated exactly? Contact the DSO at the school you want to transfer to in order to begin the process. You may need reinstatement.

Your PERM needs to be submitted at least 365 days BEFORE the expiration of your H-1B status.
