No the person does not have to leave the US. The best situation would be if the adjustment has already been approved.

Just send regular copies with the application. These do not have to be notarized. Take the originals with you to the interview.

There is no reform yet. However there has been a proposal to eliminate the family based 4th preference category.

That depends on whether the Cuban Adjustment Act applies. Check with an immigration attorney first.
