Can I H1-B transfer to a new employer after almost 16 years, without the restriction of the H-1B cap ? Please clarify.

It appears as though you will need a new petition filed and will be subject to the cap because of the great period of time that has passed.

Apply for the B-2 visa. When the visa is issued it includes B1 and B2 visa holders. That is what is confusing.

You must wait until you actually have the card before you can start working. USCIS has been slow in producing the cards. It could take three months.

Hard copies of photos are better and be prepared to let the officer keep the pictures so make copies.

Your case is progressing normally. You should however refrain from travelling if at all possible until you are approved for residency.
